EECP for Refractory Angina: Clinical Evidence from a Case Series

Enhanced External Counterpulsation (EECP) is a non-invasive, evidence-based treatment that may help patients with refractory angina—persistent chest pain despite optimal medications and when angioplasty or bypass surgery is not suitable or has not provided adequate relief.
A published case series involving 16 patients with refractory angina evaluated the effectiveness and safety of EECP. The study included patients with advanced coronary artery disease, many of whom had previously undergone angioplasty or bypass surgery or were not candidates for further revascularization.
After completing EECP treatment, patients experienced:
- Significant improvement in angina severity (Canadian Cardiovascular Society Angina Class)
- Better exercise capacity, with a meaningful increase in the 6-minute walk distance
- Reduced dependence on sublingual nitroglycerin in many patients
- Improved quality of life and day-to-day functional capacity
Importantly, EECP was found to be safe and well tolerated, with no major treatment-related complications reported in this patient group.
While this was a relatively small study, its findings support the growing body of evidence that EECP can be a valuable treatment option for carefully selected patients with refractory angina, particularly when conventional revascularization is not feasible.
